We are submitting our Donation Form for consideration in the Zeffy Awards, Best Donation Form Category.

We attribute the tremendous success of this form to its simplicity. Choices are limited to one time or sustaining donations with suggested amounts to keep it simple for donors. No ancillary questions, just an easy way to donate.

Other non-profits could easily adapt our simple form to their use.

Once we had the perfect form, we made use of every single method Zeffy provided for sharing our form. We posted to all the social media sites, emailed to our contacts, posted to our website and even put the QR Code on posters we left around town and took to the Farmer's Market. The combination of a very simple and effective donation form, along with the broad dissemination was a winner for us and could easily be replicated by other non-profits.
