This is our all purpose/general donation form which has raised $158,207 across 1,397 donations. I think the success of this form is due to our wonderful and supportive community, and also because it is easy to share and customize as needed.

As we have upgraded our web site design, we have been able to easily match the design elements in the form. This helps with our branding. The fact that the form is easy to share and "portable" through the QR code has also made the form successful.
